Engaging Patients in Advance Care Planning

Join us for the March 10th Annual DECIDE WNY Conference with keynote speaker BJ Miller, MD. Dr. BJ Miller is one of the pre-eminent speakers on palliative and end-of-life care. Drawing on his expertise as a physician, former Executive Director of Zen Hospice Project, and as a patient, he is an advocate for a healthcare system that minimizes unnecessary suffering and maximizes quality of life. Dr. Miller has lectured widely on all aspects of palliative care, as well as person-centered care, the therapeutic potential of aesthetics, and how to design for life.

 

Dr. Miller will be joined with roundtable facilitator Henry Davis from the Buffalo News and speakers Yvonne Askew; Katie Grimm, MD; Chris Kerr, MD; Sandra Lauer, RN; Laurie Menzies; Kathleen Mylotte, MD; and Charles Yates, MD.

 

Breakfast and registration will begin at 7:30 AM with the conference starting at 8:00 AM. Continuing education credits will be made available.

Heart Smart for Life Webinar

The Mercy Hospital Foundation and Mercy Comprehensive Care Center’s Heart Smart for Life is a grant-funded wellness program focuses on improving the underlying causes of heart disease for high-risk patients and residents.  Heart Smart for Life is a collaborative initiative aimed at ensuring vulnerable populations – those who otherwise might fall through the cracks – have access to nutrition education, biometric health screenings, wellness programs, and other resources focused on preventing heart disease. 

 

Outcomes include improvements in physical activity, healthy eating, and knowledge of cardiovascular health and nutrition, as well as improvements in clinical measures such as blood pressure, Body Mass Index, and cholesterol levels.  Heart Smart for Life is measurable, successful, and improving lives. 

 

Join us on March 29 from 10 to 11 a.m. to learn how you and your healthcare team can start your own about Heart Smart for Life program.  We will share lessons learned, tips and tools, and engagement tactics that are making a difference in the health of our community.

National Diabetes Prevention Program Lifestyle Coach Training

Does your organization offer National Diabetes Prevention Programs? Are you looking for more Lifestyle Coaches?

 

Through the Local IMPACT grant, there is an opportunity to train 15 lifestyle coaches! This training will take place at The Erie 1 BOCES building located at 355 Harlem Rd, West Seneca, NY 14224 on April 9th and 10th from 8:30 AM - 4:30 PM. MUST ATTEND BOTH DAYS!

 

This training is FREE. Refreshments and lunch will be provided. Space is limited and will fill up quickly, so register today!
